Shared Decision-Making is at the heart of personalised care, something all health and care professionals across all organisations can embed into their interaction with patients’, as many already do, ensuring all patients become empowered and feel involved in their own health and care planning and decision-making.
Personalised care represents a new relationship between people, professionals and the health and care system which is a positive shift in power and decision-making that enables people to have a voice, to be heard and to be connected to their communities.
